Senior Account Manager – Home Health at Health Link in Milpitas

• For over 20 years, Health Link Home Health & Hospice has served patients throughout the San Francisco Bay Area. We are seeking an experienced, results-driven Senior Account Manager to join our Community Relations team and support continued growth throughout Santa Clara County.
• Working closely with the Director of Business Development, the Senior Account Manager will focus on developing high-potential referral accounts, expanding existing partnerships, and building relationships with healthcare decision-makers throughout the South Bay.
• This is a field-based role for an experienced healthcare sales professional with a demonstrated ability to develop new business and produce results. The ideal candidate can work with a high level of independence, identify opportunities within the market, and build meaningful referral relationships from the ground up.
• You will be supported with a company vehicle and AI-powered technology designed to provide referral insights and support efficient territory routing.

Responsibilities

• Independently identify, develop, and grow high-potential referral accounts throughout Santa Clara County
• Establish new relationships with healthcare decision-makers and convert those relationships into referral opportunities
• Build and expand relationships with hospitals, skilled nursing facilities, physicians, medical groups, clinics, senior living communities, and other healthcare organizations
• Strengthen existing accounts by developing additional relationships with physicians, departments, and key decision-makers
• Identify opportunities to increase referrals and admissions from existing accounts
• Conduct in-person meetings, presentations, account visits, and strategic follow-up with referral partners
• Educate healthcare partners on Health Link’s home health services, clinical programs, and capabilities
• Execute business development initiatives and territory priorities in partnership with the Director of Business Development
• Identify emerging market opportunities, referral trends, and competitive activity and provide meaningful feedback to the Director of Business Development
• Maintain strong relationships and consistent communication with key referral partners
• Collaborate with Intake, Clinical, Operations, and other internal teams to support a strong referral and patient experience
• Meet established referral, admission, and account growth goals
• Represent Health Link professionally throughout the South Bay healthcare community

Requirements

• 3+ years of healthcare sales, business development, account management, community relations, or related experience
• Demonstrated success developing new business and growing healthcare referral relationships
• Proven track record of meeting or exceeding referral, account, or growth goals
• Ability to independently identify opportunities, develop new accounts, and grow existing business
• Strong relationships with healthcare professionals and decision-makers in the South Bay highly valued
• Strong relationship-building, communication, presentation, negotiation, and follow-up skills
• Self-motivated and comfortable working independently in the field
• Strong organizational and time-management skills
• Knowledge of the Santa Clara County healthcare market strongly preferred
• Home health, hospice, post-acute care, senior care, hospital, or skilled nursing experience preferred
• Valid California driver’s license and driving record that meets company requirements
