Medical Social Worker at Cabrillo Hospice in Orange County and Los Angeles County

Job description

• Are you passionate about supporting patients and families through life’s most vulnerable moments? Cabrillo Hospice is seeking a compassionate and dedicated Social Worker to join our interdisciplinary hospice team and make a meaningful difference in the lives of patients and caregivers.

Responsibilities

• Conduct Comprehensive Psychosocial Assessments
• Assess the emotional, social, and environmental needs of patients and families, including caregiver stress, family dynamics, coping abilities, and safety concerns.
• Provide Emotional Support & Counseling
• Offer compassionate counseling to patients and families navigating serious illness, grief, and end-of-life transitions.
• Collaborate with the Interdisciplinary Team
• Work closely with nurses, physicians, chaplains, and other care team members to develop and support individualized plans of care.
• Connect Families to Community Resources
• Serve as a liaison to community agencies and support services to ensure patients and families have access to appropriate resources.
• Bereavement Support
• Participate in and support hospice bereavement services, providing follow-up and resources to families after the loss of a loved one.
• Ensure Compliance & Quality Care
• Maintain accurate documentation and ensure services align with hospice regulations, policies, and standards of care.

Requirements

• Master’s Degree in Social Work (MSW)
• Hospice or healthcare experience preferred (geriatrics or end-of-life care a plus)
• Strong interpersonal and communication skills
• Compassionate, patient-centered approach
• Ability to work independently while collaborating within a multidisciplinary team
• Cultural sensitivity and respect for diverse backgrounds and belief systems
