Job description

• Hope For Youth's Residential Services Department provides community based, trauma informed, youth and family driven care to adolescents and young adults in Nassau and Suffolk County. Serving youth from varying points of entry with the child welfare system, juvenile justice system, and runaway and homeless youth/young adults, the opportunities to alter the trajectory for youth in need are abundant. Youth are supported 24/7 by a team of multi-disciplinary professionals, paraprofessionals, and agency administration that consistently meets standards for accreditation and licensing regulations.
• Hope For Youth's Adolescent Diagnostic and Respite Services Program ("Diagnostic") provides respite, emergency and diagnostic services for up to 12 youth, co-ed, from ages 12-20. Youth are referred through Nassau and Suffolk Department of Social Services, Probation and the Crisis Bed Network. The Diagnostic Program Manager will oversee all case management services, aftercare services, prepare psychosocial assessments and diagnostic evaluations. The Program Manger provides direct supervision to Assistant Program Manager, Case Worker and Case Worker Assistant. The Program Manager is instrumental in maintaining the overall functions of the program with the assistance of the Assistant Program Manager. The position offers the opportunity for some remote work within agency policy, including but not only completion of new hire introductory period and good standing.

Responsibilities

• To be successful in this role, you must be ready, willing, and able to:
• Function within the policies and procedures outlined in the Program Practice Guide and Employee Manual.
• Provide overall management of your assigned site.
• Complete reports, case documentation, and psychosocial assessments.
• Liaise with DSS, schools, and collateral contacts.
• Deliver crisis intervention and counseling when needed.
• Supervise, coach, and provide staff with the knowledge and tools to succeed.
• Develop discharge plans and provide 6 months of aftercare services.
• Conduct home visits as necessary.
• Perform all related tasks or other agency and/or programmatic required needs associated with this role
• Support other residential program locations and agency projects as assigned.

Requirements

• LMSW required; LCSW preferred
• 2 years' post-master's experience with child-serving systems (child welfare, juvenile justice, etc.)
• Strong computer proficiency (Microsoft Word, Excel, and reporting portals)
• Valid NYS driver's license in good standing
• Access to a reliable vehicle (mileage reimbursement available)
• Ability to provide 24/7 on-call availability during the week; participate in weekend on-call rotation
