Job description

• GENERAL DESCRIPTION: Responsible for coordinating and supporting medical-related services for individuals receiving DDS services across day, community, and home settings. This position provides health monitoring, medication-related support, medical documentation, appointment coordination, transportation, communication, training assistance, and advocacy in collaboration with the Goodwill Nurse, supervisory team, providers, families, guardians, and service teams to promote health, safety, dignity, continuity of care, and person-centered support.

By serving as a key liaison between individuals served, medical professionals, staff, families, guardians, and service providers, this position helps ensure medical recommendations are understood, implemented, and monitored effectively across service settings. This role supports regulatory compliance, reduces health and safety risks, strengthens communication among stakeholders, and helps individuals maintain the highest possible level of well being and community participation.

Responsibilities

• Participate as a member of the ISP team by attending ISP meetings and related service/team meetings as assigned; gather, organize, and communicate medical information needed for services, goals, safety plans, medical protocols, and individualized supports.
• Provide medical-related instruction, monitoring, follow-through, and support in day, community, and Shared Living settings as directed by the ISP, Goodwill Nurse, or supervisory team.
• Promote health and safety by responding to medical concerns, assisting staff, completing remote or in-person health monitoring as assigned, and using CPR, First Aid, MANDT, or other approved techniques when needed.
• Maintain and coordinate medical documentation, records, reports, appointments, follow-up tasks, meeting paperwork, medication administration records, training records, and required electronic or paper systems, including Therap.
• Coordinate medication-related processes, including pharmacy and provider communication, inventory, staff direction, monitoring, medication error follow-up, and quality review for individuals whose medication services are administered or monitored by Goodwill.
• Coordinate or conduct assigned medical-related trainings, including CPR, Medication Aide, adaptive equipment, and other delegated supports; maintain required trainer certifications, training records, and follow-up documentation.
• Review medical information, medication concerns, health monitoring results, and service-related needs; identify follow-up actions and prepare objective summaries for the Goodwill Nurse, providers, families, guardians, managers, or other stakeholders.
• Transport and attend medical appointments as assigned to support communication, documentation, advocacy, and follow-through on medical or supervisory direction.
• Serve as a primary medical communication contact for individuals, medical personnel, families, guardians, service coordinators, providers, managers, staff, and other stakeholders while maintaining HIPAA compliance, confidentiality, professionalism, advocacy, and timely follow-through.
• Represent Goodwill positively in internal, community, and medical settings by promoting inclusion, dignity, respectful communication, and person-centered support.
• Punctual and consistent attendance and appearance requirements per Goodwill policies.
• Perform work activities in conformance with Goodwill values, immediately report any wrongdoing (unethical or improper conduct, fraud, fiscal mismanagement, etc.), embrace and support diversity, and encourage and support others to do the same.

Requirements

• High school diploma or GED/equivalent required with a minimum of two years of experience as a Medication Aide.
• Must become certified as a Medication Aide through Goodwill training within three months of hire.
• Must complete and maintain assigned trainer certifications, such as CPR, Medication Aide, adaptive equipment, or other medical related training certifications required for delegated responsibilities.
• Experience or knowledge of nursing assistant procedures, rehabilitation, habilitation, or support services for persons with disabilities preferred.
• Must demonstrate strong, accurate, and objective documentation skills in accordance with Goodwill standards, HIPAA requirements, and assigned medical service procedures.
• Must be proficient with Microsoft Office, word processing, databases, electronic case management systems, and Therap.
• Must demonstrate strong verbal and written communication skills, professional phone and email etiquette, knowledge of HIPAA requirements, and commitment to confidentiality.
• Must be at least 19 years of age.
• Maintain a valid driver’s license issued in the state of Nebraska and meets all insurance requirements.
• Physical ability to perform essential safety-related duties, including CPR, MANDT, First Aid, and other assigned techniques.
• Ability to communicate verbally, use adequate speech volume, and hear sufficiently for conversation, telephone use, and emergency information systems.
• Ability to move safely and independently within work areas, service settings, medical appointments, community activities, and other assigned locations.
• Visual acuity, depth perception, and field of vision within normal range or otherwise reasonably accommodated.
• Physical stamina to work at least eight hours per day, travel as assigned, and safely lift objects up to 50 pounds when required.
• Ability to provide transportation and safely support individuals during appointments, community activities, and service-related travel as assigned.
• Demonstrate ethics above reproach by making decisions that prioritize integrity, transparency, accountability, dignity, confidentiality, and compliance.
• Adapt positively to changing routines, priorities, medical needs, service settings, and urgent or sensitive situations.
• Follow through independently on assignments while seeking appropriate direction from the Goodwill Nurse, ISP team members or applicable medical personnel, and supervisors.
• Maintain excellent organization of records, reports, appointments, inventories, training documentation, and follow-up tasks.

• Communicate effectively with individuals, staff, guardians, providers, medical professionals, and other stakeholders using clear oral and written communication.

• Use leadership skills to instruct, plan, support, monitor, and promote quality medical-related practices for people served.
